Eyelid Surgery Q & A

What is a Blepharoplasty?

Eyelid surgery, or blepharoplasty, is a surgical procedure to improve the appearance of the eyelids. Surgery can be performed on either the upper or lower lids, or both. Whether you want to improve your appearance or are experiencing functional problems with your eyelids, this procedure can rejuvenate the area surrounding your eyes.

Specifically, eyelid surgery can treat: Loose or sagging skin that creates folds or disturbs the natural contour of the upper eyelid sometimes impairing vision, excess fatty deposits that appear as puffiness in the eyelids or bags under the eyes, drooping lower eyelids that reveal white below the iris (a term known as scleral show), or excess skin and fine wrinkles of the lower eyelid.

This is one of our favorite procedures to perform because the eyes are the window to the soul and eyelid rejuvenation can result in a more youthful and more rested appearance. It is not uncommon for patients to hear from coworkers and friends that they look more rested and healthier post operatively after an eyelid surgery or blepharoplasty. What are the different types of blepharoplasty?

Eyelid surgery can be performed on the upper or lower eyelid and additionally there is a specialized Asian eyelid surgery. The anatomy for these procedures is very complex which is why we insist on tailoring procedures custom to each patient.

There are three fat pads in the lower eyelid and two in the upper eyelid that can give the appearance of fatigue or a full appearance. Typically, the upper eyelid rejuvenation involves medial fat pad removal only, but the lower eyelid involves all three fat pads being removed. There are important ocular muscles in these regions that are carefully approached to ensure that no trauma occurs. We also specialize in Asian eyelid surgery, which can present a difficult situation to a novice surgeon because anatomically the muscle that elevates the eyelid is positioned lower than normal. If this is recognized and the surgery is customized to the specific patient, this does not pose a problem to a surgeon with experience in this subset of patients.

What Can I Expect After the Procedure?

Most patients go home within a few hours after the procedure. Watering eyes and blurred vision from the ointment used in the eyes are typical and can last upwards of one to two weeks.

Swelling, bruising and redness are also normal. Patients who use contact lenses will not be able to wear them for about two weeks. All patients must sleep with the head elevated to reduce swelling, which usually gets better within a week or two. The scars from the surgery will take about six months to fade although in the lower eyelid surgery there is typically no scar.
